Note that our membership year runs from April 1st until March 31st the following year.

Application form (pdf)	Membership application form in pdf format.

Application form (Word)	Membership application form in Microsoft Word format format. Open in Word and enable editing to fill it in.

Forms can be downloaded and completed on-screen. On completion, save it and email it to the membership sec. at members@ashbyu3a.co.uk

Or forms can be printed out and filled in by hand and either posted or handed to the Membership Secretary or scanned and emailed back.

Documents completed online need not be signed if they are emailed back.
